The forms you’ll need are available for downloading below. Start with the First Appointment Prep Form. The remaining forms are parts of your Intake Packet. Simply fill them out as best you can and then, contact us to schedule an intake appointment.
Treatment plans are individualized so cost of the full course of your treatment will depend on your therapeutic needs. We will work together in coming up with the right plan for you. For specific details about the cost, please refer to the Treatment Process & Fees Handout, available as a download below.
Cash, Credit Card, Paypal, Venmo, or check are accepted for payment at the time of service. Please fill out the Credit Card authorization Form below unless we’ve made other arrangements for payment.
These treatments are not covered by most insurance companies. It’s possible some companies may reimburse for the ‘integration sessions’ when you submit receipts.
If you do not attend your scheduled appointment, and you have not notified us at least 48 hours in advance, you will be required to pay the full cost of the session. Details & caveats are in the Process & Fees handout below.
Current COVID Policy: We ask that all patients wear masks in the waiting areas. You and your clinician may determine whether you will wear masks while in the treatment rooms. All treatment rooms have air filtration units.
